Mental health social work

Working in partnership to support recovery for people with mental health needs.

We work in partnership with people who use Mental Health Services, carers, NHS and other organisations to ensure that a range of services are provided to support recovery for people with mental health needs.

The Mental Health Social Work Team

We have two Mental Health Social Care Teams, one covering the North of Worcestershire and one covering the South of Worcestershire both are made up of social workers and social care workers. 

Each team supports people make connections to their neighbourhood and community, as well as considering whether more formal care and support is required under Care Act 2014. Visit our assessments and eligibility page for more information. 

There is a social worker available on the phone every day, to have a conversation with people who need some support to live a good life due to mental health difficulties. 

What we do

  • we work closely with the Neighbourhood community Mental Health teams
  • we work closely with Mental health inpatient and rehab units to support discharge from hospital back into the community
  • we work children's services in partnership to ensure effective transitions to Adulthood
  • work with individuals to make transitions to alternative accommodation, for example into supported living or their own tenancy.
  • complete reviews of care and support plans to ensure they are continuing to meet assessed needs and outcomes. 
  • in conjunction with the Integrated Care Board we are responsible for ensuring that any section 117 aftercare needs are met

If you are  struggling with your mental health which you feel is  not improving with self help tools, the first thing you should do is speak with your GP, or if you have one, your Lead Practitioner in your Community Mental Health team.

Mental health issues are common and treatable. The sooner you seek help, the better. Please also check out the support available both locally and nationally.

Approved Mental Health Professional Service

The Approved Mental Health Professionals (AMHP) Service is responsible for undertaking assessments under the Mental Health Act 1983.

Section 117

Section 117 of the Mental Health Act 1983 (as amended by the Mental Health Act 2007) places a joint legal duty on Integrated Care Board (formally CCG) and the Local Authority to provide (or arrange) the provision of aftercare for individuals who have been detained under specified sections of the Mental Health Act once they leave hospital.

The Mental Health Social Work teams work closely with the individual and the Neighbourhood Mental health teams to ensure that eligible s117 aftercare needs are met.
