
Learning Disability and Autism, support to avoid hospital admission
Information about the Learning Disability and Autism Admission Avoidance and Discharge (LDA-AAD) Team.
Who we support
We support children and young people with a diagnosed learning disability and/or autism who have serious mental health needs. If they are at high risk of needing specialist mental health hospital care, they can be referred to our Learning Disability and Autism Admission Avoidance and Discharge Team (LDA-AAD Team).
Referrals can be made by:
- health professionals
- schools
- social care services
- the child or young person themselves (self-referral)
How we help
If a child or young person is eligible, they will be added to the Dynamic Support Pathway (DSP), which helps manage their care.
They will be given a Red, Amber, or Green (RAG) rating to show their level of risk for hospital admission.
We work with Health, Education, and Social Care teams to:
- make sure they get the right care at the right time
- prevent crises
- reduce the need for hospital admission
Consent and referrals
To access this support, the child or young person (if able) or their parent/guardian must give consent. If they cannot consent, a decision will be made in their best interest.

Care, Education and Treatment Reviews (CETRs)
We also organise Care, Education and Treatment Reviews (CETRs). These reviews check that the right support is in place and help to remove barriers that may stop progress.
The main points are:
- anyone can request a CETR
- being on the Dynamic Support Pathway is not required, but referrals may take longer for those who are not
- CETRs do not decide if someone should be admitted or discharged from hospital

System Navigators, helping you through the system
Our System Navigators support children and young people with high-risk RAG ratings (Red or Amber).
They:
- listen to the child, young person, and their family
- help them understand and access services
- make sure their voice is heard when decisions are made
- provide ongoing support even after they leave the Dynamic Support Pathway
