Reporting Early Help Youth Support

How to report Early Help provided to young people by youth organisations.

What is Early Help?

Early Help is providing help and support to a child, young person or their family as soon as it is identified that they need additional help and support.

When Early Help has been delivered to a young person, to ensure joined up working with other professionals, youth organisations are asked to complete an Early Help Action Report and upload it to the portal below:

Third Party Professional Assessment Form

When the child/young person, their family, and the Team Around the Family (TAF) agree that the needs have been met, the plan will end and all those involved should be informed. 

Information about bringing together a Team Around the Family (TAF) can be found here: Team Around the Family information and guidance

A closing summary should then be completed and submitted to the Early Assessment Portal.
